 Mode Effect Analysis/Human Factors Enginnering 
 and Root Cause Analysis; (4) understand the 
 concept of "Fail Safe Team"; (5) understand 
 the principles of "strong solutions" to error 
 prevention; and (6) understand barriers to 
 culture change and to a culture of safety.\n\n
